Montefiore Makes Housecalls to Homebound Seniors

Montefiore Medical Teams Care For Bronx Elderly In Their Homes

Specialized medical teams from Montefiore Medical Center are making housecalls to elderly Bronx residents who have difficulty getting themselves to doctors' offices for medical care. This program is unique in the Bronx and one of the first of its kind in the New York Metropolitan Region.

"Our goal is to provide quality primary care for the elderly where they need it, and that means offering state-of-the-art diagnosis and treatment in their homes," said David Bernard, MD, CMO medical director, Montefiore. "In addition to delivering high quality long-term care, our programs also offer urgent house calls on a same-day basis. We think this addresses the growing healthcare needs of an extremely vulnerable population," said Dr. Bernard.

The Montefiore medical teams consist of physicians, nurse practitioners, social workers and key support staff.  "By increasing access to primary care for the frail elderly, we can play a role in helping prevent conditions that would bring these patients to the hospital with more serious health problems," said Laurie Jacobs, MD, head of the Unified Division of Geriatrics, Montefiore.

Montefiore Medical Center has established community partnerships as the foundation of this program with the New York City Housing Authority, Fair Pharmacy, Montefiore Home Health Agency and MetroCare Ambulance Service.

The program is available to eligible seniors in the Bronx, whether or not they live in senior-funded housing, housing authority buildings or private homes and apartments.
