Physician Assistant Interns
PLEASE BE SURE YOUR PROGRAM IS SPONSORED BY ALBERT EINSTEIN COLLEGE OF MEDICINE AND LOCATED AT CHAM, MOSES OR WEILER BEFORE COMPLETING THIS SECTION.
Please print or type clearly on all applications and submit all required documents and forms to the House Staff Office, not to your department. If you would like to come in person to submit your documents and forms, call for an appointment.
- Please note that we must receive completed forms and required documents in order to put you on payroll.
- Your contract for the 2009-2010 year, along with our background information form, will be emailed to you. Please sign both and return it to the House Staff Office.
- If you are on a VISA please call the House Staff Office immediately to start the process.
- In the event that your current address changes before July, please submit a Change of Information Form immediately.
- In the event that your email address changes please notify the House Staff Office immediately.
Credentialing Tips

When applying for your limited permit, please make sure that you add Montefiore Medical Center and Jacobi Medical Center as your practicing institutions.

Make sure that you obtain a letter from your school stating either the anticipated date of graduation or degree conferred on a specific date. (A notarized copy of your diploma to our office to satisfy this requirement.)

An Infection Control Certificate is required for all incoming PA Interns. You can go to http://www.proceo.com/ and obtain the certificate for a $50.00 fee.

If you are BLS and ACLS Certified, please make sure that the cards are not expired. IF they have expired please visit http://www.americanheart.org/ in order to find the nearest center for recertification.

The registration certificate has to be signed by the applicant above the licensee/registrant space.
Reminder: The American Heart Association or the Red Cross in the continental USA must issue BLS & ACLS.
