You have a number of rights guaranteed to you as a patient at any one of Montefiore's hospitals. Understand these rights, consistent with New York State law. Our Nurses

Medical Records

Primary care patients can email their doctor, check on lab results, request a medication refill, access information to manage their health and create their own personal health record to access at any time 24/7 through a secure patient portal. Learn more about this online feature.

Applicable Fees and Delivery Expectations

In accordance with New York state law, there is a per page fee ($0.75 per page and $6.50 for medical records on CD) if the medical record is being released to the patient or patient representative. If copies are going directly to a physician or hospital, there is no charge.

Patients and authorized patient representatives should allow 7 to 10 business days to process the request.

To request copies of X-rays or other radiological images

If Radiology images are needed please contact the Radiology Department at 718 920 6697 and they will assist you on how to obtain the images requested.

If Radiology reports are needed it can be requested by mail or in person by visiting a Montefiore Hospital site Health Information Management Department.

Amendments

A request to amend records can be done in person by visiting a Montefiore Health Information Management (HIM) Department.

2. Wakefield HIM- 600 East 233rd street, Bronx, NY located on the 1st floor ( please ask security to guide you there is no room number for this location)
3. Westchester Square (MWS)- 2475 St. Raymond Ave., Bronx, NY located on the 2nd floor, room number 2-9
4. Moses HIM- 3411 Wayne Ave, Bronx, NY located on the 6th floor

Financial Services

Insurance Plans Accepted

Montefiore also accepts most types of health insurance plans, HMOs and managed care plans. View some of the most common plans accepted at Montefiore.

Pay Hospital Bills Online

To help patients deal with the stressful and overwhelming experience of paying their medical bills, Montefiore has created a Patient Payment Portal, which allows for hospital bills to be paid online. The site also contains information to help patients understand their bills and Montefiore's billing policies, while also helping them to apply for financial aid. Healthcare Proxy and Living Will

Montefiore recognizes a patient's right to make health care decisions and to formulate an oral or written advance directive such as a Health Care Proxy or a Living Will.
