Home Health Nurse
NYS RN license and current registration plus previous experience required. Ability to provide own transportation. Provide direct nursing care to patient in their home. Previous Med/Surg, Geriatrics experience preferred. Rotations may be required.
Home Care Coordinator - Long Term
NYS RN license and current registration required. BA in Nursing and one (1) year experience in a certified home health agency or two (2) years hospital experience. Provides professional nursing management and coordination of the patient's plan of care. Assures services provided and within the fiscal parameters of regulatory guidelines. Interacts with local regulatory agencies and performs all duties and activities in accordance with agency policy and procedures. Certified home health or long term care experience and bilingual preferred.
Patient Service Manager
Master's Degree in Nursing, NYS RN license and current registration, five (5) or more years management experience plus three (3) or more years clinical experience in related field required. Manages the day-to-day operations of the department: Supervises, trains and evaluates staff.
CHHA Liaison
BA in Nursing and two (2) years recent medical surgical experience in a hospital setting. NYS RN license and current registration required. The Liaison Coordinator assesses, plans, and coordinates home care plans for clients in accord with professional standards, regulatory guideline, and Agency policies. Provide consultation, education and interpretation of programs and services to the general public and referral sources.
Fee For Service Nurse
NYS RN license and registration required. Ability to provide own transportation. Conduct initial and ongoing patient assessments in the home. Experience in the following area is preferred: Med/Surg, Psych, Infusion, Pediatrics, AIDS. Bilingual a plus.
